Contract Lab and QC Staffing Request Form Template
One page that tells a staffing firm the bench, the rules and the shift before any resume.
No form for the PDF. The editable Word file takes a work email.

A lab staffing request should name methods and instruments, sample matrix, GMP or GLP, shift, gowning, start date and duration, not just the job title. This one-page template captures all of it, with KORE1 reference values.
Last updated: November 1, 2026
What would you send a staffing firm if a QC seat opened tomorrow? If the answer is a job title and a date, the firm has to guess the rest.
And guessing is expensive. “QC Analyst II” could mean HPLC on a stability bench or plating samples in a cleanroom, and those two people rarely apply for each other’s jobs. You can’t judge the first resume against a bench nobody described. Nobody can.
So send the bench. This is the form I’d want from any lab manager asking me for laboratory staffing or quality control staffing, and it sits inside our wider life sciences staffing work on contract staffing terms. The reference values come from 54 KORE1 lab and QC contract placements, with a 193-day median assignment and a middle-half bill range of $33 to $56 an hour, so you can put real numbers on the form before anyone quotes you.

| Field | Why it matters | Example entry |
|---|---|---|
| Seat title and level | A title only starts the conversation, because one title can cover very different benches. | QC Analyst II, stability |
| Site and lab area | The commute decides who says yes, and the lab area decides what else on this form applies. | Irvine, QC stability lab |
| Who approves the resume | A named person gets a resume read and answered faster than a shared inbox does. | QC manager, plus one senior analyst |
| Field | Why it matters | Example entry |

| Methods and instruments | Running a method and having watched it run are two different resumes. | HPLC with Empower, dissolution, Karl Fischer |
| Sample matrix | Prepping a suspension is not the same job as prepping a clean API solution. | Finished tablets and stability pulls |

| GMP or GLP | They are separate rulebooks, and the answer changes the records, the training and the people who fit. | GMP, commercial release testing |
| Gowning | It sets who can walk into the area, and how long it takes before they can. | ISO 7 gowning, qualified on site before area entry |
| Training before bench work | A GMP seat needs training in the particular operations the person performs, so plan for it. | SOPs read and trainer named before the first sample |

|---|---|---|
| Shift | A second-shift seat narrows the list before anyone looks at a method. | Second shift, Tuesday to Saturday |
| Start date | A date only counts if a trainer and a gowning slot are booked for it. | First Monday next month, gowning slot booked |
| Duration | A length tells a candidate what they are leaving a job for. No idea yet? Start with the median. | 193 days, the KORE1 lab and QC median |
| Field | Why it matters | Example entry |
|---|---|---|
| Contract or contract-to-hire | It changes who says yes. A KORE1 contractor can convert at no fee after 1,040 hours. | Contract-to-hire, review at 1,040 hours |
| Budget reference | A range saves a round of resumes you could never have approved. | $33 to $56 an hour, median $40 |
The example entries show the level of detail, so replace them with your own. The dollar and day values are KORE1 reference figures as of October 2026, not a quote.
The method grid is the part I’d fill in first. Mark what the person must run alone on day one, what you’re willing to teach and what isn’t on this bench at all, then swap my ten methods for your own, because a grid full of the wrong methods tells a recruiter nothing. Writing the seat itself? Start with the QC analyst job description template, because a job description sells the seat to a candidate and this form briefs the recruiter.

The filled squares
Which Five Fields Decide Whether the First Resume Fits?
Methods and instruments, GMP or GLP, shift, gowning and start date. Get these five right and the first resume can actually be judged.
- Methods and instruments. Say whether the person runs the method or supports it, and name the software too. HPLC with Empower is a narrower search than HPLC. That’s the point. Your lab records already have to state each method used under 21 CFR 211.194, so the list exists somewhere.
- GMP or GLP. Separate rulebooks. 21 CFR Part 58 covers nonclinical laboratory studies, while Part 211 covers making and holding finished drugs. Good resumes often show one and not the other, so read up on GLP vs GMP experience before you call them interchangeable.
- Shift. Second shift removes people from the list before anyone checks a method. Better to learn that now. Not at the offer.
- Gowning. A lab coat and glasses is one conversation, while full cleanroom gowning with a qualification step is another, and some very good chemists would rather keep out of the gown for the rest of their careers. For an aseptic area, FDA’s aseptic processing guidance, which is nonbinding, says training should come before an individual enters. Ask early.
- Start date. 21 CFR 211.25 requires training in “the particular operations that the employee performs,” so on a GMP seat the first day isn’t the first sample. Write the date you can receive someone, with the trainer booked. The training records a GMP contractor needs shape that date too.
The other eight fields sharpen the search. These five sort it.

Budget and Duration Reference Values for the Form
“What is your rate?” I get that one more than any other, and it deserves a number. KORE1 lab and QC contract seats bill a median $40 an hour, and the middle half sit between $33 and $56.
Put that range on the form as a reference, then let the bench move it, because an LC-MS method specialist on second shift won’t sit where a general lab tech on days does, and a firm that can see your range can say so before the resumes arrive. Want your own numbers? The lab and QC cost calculator runs them by seat and length.
| Length | At $33 | At $40 | At $56 |
|---|---|---|---|
| 3 months520 hours | $17,160 | $20,800 | $29,120 |
| 6 months1,040 hours | $34,320 | $41,600 | $58,240 |
Hourly bill rate times hours at 40 hours a week, a reference and not a quote. Source is KORE1 placement data from 2019 to 2026 for the life sciences, lab and QC family, n=54 W2 contract placements, 14 of them since 2022 with the same $40 median. Anomalies and payroll-service deals are excluded. The full dataset is 79% California. This family isn’t a cell in the Contract Rate Index, which covers IT, ERP, data, engineering and accounting. See lab and QC bill rates for more on the range.
“This is just a hiring manager with a budget, going outside and hiring something and saying, I have this to get done and I need you to do it.”

That’s the whole job of this form, really. Say what needs doing. Say what you have to spend. Your procurement and HR steps still apply, so run it past them the way you would any other request.

Field twelve
Contract or Contract-to-Hire on a Lab Staffing Request?
Pick contract when the work has an end, like a release backlog or a stability pull that needs covering. Pick contract-to-hire when the seat will outlast the project and you want to watch someone run your methods before you make an offer.
The arithmetic is friendly here. A KORE1 contractor can convert to your team at no fee after 1,040 hours, which is 26 weeks at 40 hours a week, and the median lab and QC contract runs 193 days, about 27 and a half weeks. So the typical assignment reaches the conversion line right around the time it would end. Handy.
Decide on the form anyway. Somebody who wants a permanent job and somebody who wants six months of varied work are different people, they answer the first call differently, and a recruiter who knows which one you need won’t waste an afternoon of either. If you’re unsure, write contract-to-hire and say why, because how contract-to-hire works is worth ten minutes before anyone interviews.
After you send it
What Happens After You Send the Request?
Nothing mysterious. Four steps. In order.
- I read the bench first. Methods, rulebook and shift come before the title, because that’s where the search actually starts.
- I call you about the blanks. A blank field is fine. It just means a short phone call before the search and not a wrong resume after it. The longer version is what a recruiter needs from a hiring manager.
- We screen against your form. A candidate who has run your method on your shift goes ahead of one whose resume merely mentions it.
- Offer, then start. The KORE1 median from offer to start is 3 days across all roles (n=1,997), but that’s a median and not a promise, and your own GMP training, gowning slot and trainer’s calendar set the real first day.

Questions
What Lab Managers Ask
What information should I give a staffing agency to fill a contract QC analyst role fast?
Give the bench, not just the title, which means methods and instruments, sample matrix, GMP or GLP, shift, gowning, start date, duration and contract or contract-to-hire. Speed comes from nobody having to ask twice.
How can I tell whether an agency can really find QC analysts with HPLC and LC-MS experience?
Listen to what they ask you back. A firm that knows the work asks about the instrument, the software, the matrix and whether the person develops methods or only runs them. If the first question is the bill rate, keep looking.
What should I budget for a contract lab or QC analyst?
$33 to $56 an hour is the middle half of KORE1 lab and QC bill rates, with a $40 median across 54 W2 contract placements. Three months at those rates comes to $17,160 to $29,120 at 40 hours a week. Treat it as a reference, because method depth, shift and metro all move the number, and 79% of the full KORE1 dataset is California.
How long do contract lab and QC assignments usually last?
193 days is the KORE1 median, a little over six months. Use it as the default if you don’t know yet.
Can a contract lab tech convert to a full-time employee?
Yes, and with KORE1 there’s no conversion fee after 1,040 hours, which is 26 weeks at 40 hours a week. Mark the seat contract-to-hire on the form if that’s the plan, so candidates hear it on the first call.
